In hypocalcemia, the total calcium level in the blood is below the normal reference range, typically below 8.5 mg/dL (2.12 mmol/L) for adults. Calcium is essential for many bodily functions, including muscle contractions, nerve signaling, and bone health. When calcium levels are too low, it can lead to various symptoms.

Hypocalcemia symptoms can vary in severity, depending on the individual and the duration of the condition. Neurological symptoms include increased muscle twitching, tingling sensations around the mouth or in the hands and feet, and muscle cramps. Seizures can occur, sometimes as the first noticeable symptom. Cardiovascular signs may include an irregular heartbeat, fainting, or low blood pressure. Chronic hypocalcemia can cause psychiatric symptoms like anxiety or depression and vision problems.

What Are the Causes of Hypocalcemia?

Low serum calcium concentrations are most often caused by disorders of PTH, or vitamin D. Low levels of PTH can be caused by genetic disorders, abnormal PTH synthesis, or postsurgical conditions such as thyroid or parathyroid surgery. Autoimmune disorders and parathyroid gland infiltration are also common. Additionally, vitamin D deficiency, renal disease, acute illnesses, and medications affecting calcium metabolism contribute to hypocalcemia.

What Are the Treatments and Therapies for Hypocalcemia?

The treatment of hypocalcemia entails calcium supplementation as well as addressing the underlying cause. Mild or chronic cases are managed with oral calcium supplements like calcium citrate or calcium carbonate. For severe cases (serum calcium ≤ 7.5 mg/dL), intravenous calcium gluconate or calcium chloride is used with close monitoring. Treating the underlying condition, such as hypoparathyroidism or vitamin D deficiency, is essential, along with addressing factors like loop diuretic use or hypomagnesemia that may contribute to low calcium levels. The goal is to correct calcium levels and prevent complications.
