What Is Cushing’s Syndrome?
Cushing's syndrome occurs when there is an excess of cortisol, a hormone produced by the adrenal glands. Cortisol has many important functions and is necessary for life; however, an excess of this hormone has negative effects on the body. Today, virtually all people with Cushing's syndrome can be treated effectively, and many can be cured.
Cushing's syndrome symptoms are caused by an excess of cortisol and typically worsen over time. Common symptoms include progressive weight gain, fragile skin prone to bruising, irregular menstrual periods in women, excess body hair, and acne due to increased androgen levels. Muscle weakness, bone loss leading to osteoporosis, and prediabetes are also common. Patients may also experience high blood pressure, cardiovascular disease, and psychological symptoms such as depression and insomnia.
What Are the Causes of Cushing’s Syndrome?
Cushing's syndrome can occur for several reasons. Hypercortisolism is most commonly caused by prolonged glucocorticoid therapy. In some cases, it happens because the pituitary gland in the brain makes too much of a hormone called ACTH, which tells the adrenal glands to produce too much cortisol. Sometimes, tumors outside the pituitary gland, like in the lungs or pancreas, can make ACTH on their own, causing the adrenal glands to overproduce cortisol. Rarely, tumors in the adrenal glands themselves can release too much cortisol.
What Are the Treatments and Therapies for Cushing’s Syndrome?
For patients with Cushing's syndrome caused by taking excessive glucocorticoids (like prednisone), we lower the dose or switch to a different medication. A team of specialists, including an endocrinologist, will manage the treatment. Surgery is the primary treatment to remove any tumors causing the problem. If surgery isn't enough, medications can help reduce cortisol levels or block its effects. After surgery, some patients might need to take glucocorticoid replacement therapy for life. Patients should keep up with vaccinations and manage any other health issues like diabetes, high blood pressure, or bone loss.
